Notice of Availability of Form 1095-C
Rice CISD has prepared the annual Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, for employees as required by the Affordable Care Act (ACA).
The District is not required to automatically mail Form 1095-C to all employees. A copy of your Form 1095-C is available upon request at no cost.
The Form 1095-C provides information about the health insurance coverage that was offered to you during the calendar year. This form is not required to file your federal income tax return. However, your tax preparer or the Health Insurance Marketplace may request it in certain situations.
